All summer-campers, please print, fill out and have parent/guardian sign the following forms before arriving at Latham Springs Baptist Camp.

Visitors using ropes or paintball courses need these forms too!

All adult leaders (18 and over) must view the following video and complete Leadership Screening form as well.

Note: If you viewed the video last summer you do not need to view the video and complete the test again this summer.
